MUST use the UNIVERSITY's checkk? 2. who can tell me where I SHOULD pay to ? both the $ 690 and $1000 to the same place? pay at the same time? 3. Form 129 and 9035 both mail to the same place Vermont ?or first mail 9035 to Labor Dept.,then after got approved,mail to Verrmont center?
