Xiaobaitu and others,

1. On the Form I-94, item 8, "Country where you live -- Lawful Permanent Residence", what should I fill out, U.S. or China? (I have lived in the U.S. for 12 years with current 485 pending.)
2. Item 10 and 11, "City where visa was issued" and "Date issued", I'm on 485 pending and come back with AP, no visa. What should I fill them out?
