I went Canada a couple of times. When I came back USA, the entry port office just looked my passport and I-797 and let me go. Looks like no record was made. Should I use this entry to answer "Date of last arrival" and "Place of last entry into the U.S.", or should use the one noted on I-94? Thank you for help.

没有换I-94的可以不用写
