请问I-765表第11项，关于以前得到OPT 陈必胜2007-08-08 08:23:12

以前申请过OPT并得到EAD 卡.
问题是我没有保持批准的信件,也不知道是那家USCIS Office,何时批准的.
我根据那个EAD卡上的Receipt Number",在USCIS网站上查case status seach. 结果显示说"On September 28,2005, we mailed you a notice that we approved this I765 Applicaiton for employment Authorizaiton. .......

把这个打印出来可以吗? 是不是可以填September 28,2005为批准date?

OPT EAD card doesn't count as 485 EAD,you can fill in NO

if you fill yes, it can just copy your OPT card, it is enough
