The USCIS published a new medical report form of I-693 and the old form is announced to be acceptable only until 05/01/2008.

This form is used by the USCIS designated civil surgeons for the permanent residence applicants. This short notice of the changed form however presents a problem. The first problem is that as the USCIS admits, there are a huge number of such qualified civil surgeons throughout the country and for that matter, throughout the world, performing medical examination for immigrants, but the USCIS is currently experiencing some problem giving a notice to some civil surgeons via emails because they did not provide their email addresses to the USCIS.

Accordingly, there may be incidents that some civil surgeons may issue the I-693 medical report on the expired form which will no longer be acceptable to the agency. The applicants have no way to learn whether the civil surgeon used the outdated version of the form since they are given it in a "sealed" envelope.

Considering such problem, the USCIS has been recommended to consider extension of acceptable date of the old medical form, which the USCIS took under advisement. Accordingly, there is some chance that just as the yesterday's announcement on EAD form, the USCIS may or may not release a similar announcement in the very near future. However, considering the fluid situation, it may be advisable for the people to print out the new form and carry it to the civil surgeon's office on the date of their medical examination appointment.
