I am now filing EAD(I-765).

I have a question about I-765:

line 14, "maner of Last entry" should be "visitor"(J2)?
Line 15, "Current immigration status" should be pending AOS or H1?if the dependent is H4, the "current immigration status" should be pending AOS, visitor or H4?

Should I enclose the J waiver approval notice with I-176?
