my case is : I came to USA at Nov./18/2005, hold J1 visa. on August/18/2006 I back China, then Sep/12/2006 came to USA again (STILL j1 VISA, as J1 visa no need apply new visa in one year) , then this year I got H1 visa ,also a new I-94 number, different from J1 visa I-94 number.

now I need fill out the form I-140, write the first J1 came time and first J1 visa attached I-94 number ? is it right ???
