上面这个东东是
Household Register Form吗？我的Title的翻译行不行？还是一定要翻译成Household Register Form？

我出生在新疆大沙漠，有史以来没有人翻译并公证这个东东，所以公证员让我自己翻译。急，恳请帮忙。谢谢谢谢
