究竟什么情况写N／A或是留空白呢？很糊涂
比如middle name， 曾用名，USCIS online account number等等

这五年中我回国两趟，前后总共待了共7（4+3）个月（其中有6个月是在收到绿卡后，我是第一次回国后人在国内的时候绿卡批准的），也需要写地址吗？
