We've received the request for medical exam from buffalo. But we have never got the instrution. As far as I know we need the Section A of the form to take the exam. Has anybody been in the same situation before? We faxed and mailed to the office, but havent got any reply yet. Also the dealine is 60 days. Could anybody throw in any suggestions?
