Question about entry date in I-140 form, thanks! yrd2006-08-04 22:06:48

I came to the US on 06/28/02 holding J-1 visa and went to Canada by car for a trip on 9/10/02. I came back from Canada by car on 10/2/02. So I did not need a new visa for re-erntry into the US and there was no stamp in my passport. Could someone tell me wihch date should I fill in the I-140 as the entry date, 06/28/02, or 10/2/02? Or I should fill in as "1st entry on 6/28/02, re-entry on 10/2/02"? Thank you so much!
