关于 Obtaining a Police Certificate Fee

1。我和妻子到当地(Pheonix) police department 打 fingerprint 时， 是否要带上blank FBI fingerprint form? 需要付多少钱？By check, money order or credit
card?
2。当我把我和妻子的指纹寄到下面的地址时 我应该买一张 18 美元 的 money order 还是 36 美元 的money order ? 我们这样做算一个request 还是两个requests? (From other source: Each written request for production of an identification record must be accompanied by a fee of $18.00 in the form of a certified check or money order, payable to the Treasury of the United States.)
