Part 4, ques.3, do you need put your chinese name and address in separate paper when you mail your supporting materials?

Part 4, ques. 6, since you have filed niw , so you need put information here. However, ther are two boxs appeared below the question when efile, my ques. is where shoud I put this information? both of them?
