Please note: all the documents that you must be in English or if foreign documents you need attached translations.
___ 1. Audit Worksheet. Do not worry about Alien Registration Number we will fill it out. #3 to #6 should be your address that you will live there for at least 2 year. Changing address after filing I-485 is possible but notices from USCIS may get lost in the mail;
___ 2. Two passport photos for I-485; another two for I-765 (EAD card) and another two for I-131 (Advance Parole). Write name with pencil on the backside of each photo; (All photos have to be passport photos).
___ 3. Down load Form G-325A (not required for children under 14) from www.uscis.gov, Immigration Forms. Please draft on a copy before you fill out this form. Leave the Items of “FILE NUMBER” and “Alien Registration Number” blank.
Note: when filling your employment history in US, please look at your employment authorization such as EAD, OPT and H-1. Make sure your statement is consistent with your documents.
___ 4. Medical Examination should be filled out by USCIS authorized doctors. ___ 5. Form I-134 for each of your dependents (not required for your spouse if he/she is working with an employment verification letter, but we need a employment letter).
___ 6. Statement of visa status including date of entry and date of change of status, Please make a copy and attach all documents which issued by the USCIS to prove your statement, including passport (all pages) visa, I-94, I-20, IAP-66, Optional Practical Training Card, Employment Authority Card, two copies of each visa page , H-1B and H-4 approval notices, F-1, F-2, J-1 waiver, etc., a entire copy of your passport. if applicable (please arrange these supporting documents in chronological order), Please note that we need 3 color copies of passport page (only the page that has your picture on it and shows your passport is valid); Also make sure you provide the copy of you I-94 for the last time you enter the U.S.
___ 7. Copy of Birth Certificate (must have your name, date of birth and parent name) if not, 2 affidavits from older relatives (parents preferred) documenting you date of birth and a non-availability certificate from the government where you were born.
___ 8. Evidence of marital status (Marriage License, Divorce Certificate, if applicable), please three copies for each of your family members (spouse and children).
___ 9. Employment letter from your employer to verify your employment should indicate your title and current salary (not required if concurrent filing I-140 and I-485);
___ 10. Checks payable to USCIS: For filing I-485, $395.00 ($325.00 + $70.00 Fingerprint fee) for each adult, $225.00 for each child under 14; $180.00 for each EAD card; $170.00 for each Advanced Parole; Note: We do not charge attorney fee for EAD card and Advance Parole when filing together with I-485 for the first time. We do charge $100.00 for each EAD card or Advance Parole application after I-485 has been filed. (The USCIS prefer you write separate checks for each application.).
