1. for question 15: home address. Here I should put down my chinese address or USA address?
2. for question 22: when to arrive USA. Here I put down 9/21 or 10/1 since my flight will be on 9/21
3. for question 24 : at what address will you stay in the US? Here I put down my company's address or my usa home address?
4. question 25: name and telephone number of person in usa who will be staying with or visiting for business?
here I put down my boss' name or my hu*****and's name since my purpose of the trip is to work in usa
5. question 26: how long do you intend to stay in the usa? I put down 3 yrs??
