I filled in both I-94 'arrival' and 'departure' record cards. Like you mentioned, I keep the 'departure' record card. For the 'arrival' record, I have several blanks which I am not so sure, they are how I should fill in the blanks ' 8.country where you live', '10. city where visa was issued' and ' 11. date issued'. My wife is also going to use AP to enter US. Appreciate your help and time.
