Help on I-140 form fishinthesea2007-07-29 15:25:52

I am filling in my I-140 form. I have 3 questions that I am not sure.

1) Did you file ETA-750B (in duplicate)? I cannot even find it online. Did you file ETA 9089 instead of ETA-750B?

2) In Form I-140, part 6: "Address where the person will work if different from address in Part 1", what did you write?

3) As individuals, do we need to write IRS#? I am thinking that it is only for sponsoring companies.
