2.Personal check in the amount of $395.00, the I-485 filing fee ($325) plus the biometrics services fee ($70);
3.Two photos, in an envelope marked with my name and social security number on the back of the each photos, and attached to the front of Form I-485;
4.Form G-325A (Biographic Information) 4 pages, duly filled;
5.Form I-693 (Medical Examination) in a sealed envelope;
6.Photocopy of the Form I-140 (Immigrant Petition for Alien Worker) filing receipt notice (receipt #SRC-xxxxxxxxx) for the primary applicant (my hu*****and xxxxx);
7.Photocopy of my notarial birth certificate with certified translation;
8.Photocopy of my notarial certificate of marriage with certified translation;
9.Photocopy of my passport, with all pages with any stamps/visas/marks;
10. Photocopy of my Form I-94, Nonimmigrant Arrival/Departure Record;
11. Photocopy of all my past I-20 forms;
12.Photocopy of my EAD card for F-1 Optional Practical Training;
13.Photocopy of Income Tax Returns for last 3 years and W-2 Form;
14.Photocopy of my diploma (M.S);
15.Photocopy of my driver’s license;
16.Signed statement that copies of documents submitted are exact unaltered photocopies of originals.
In this application, I am not including an employment letter and most recent 3 month’s payslips, since I only worked part time as a xxxxxxx(using my Optional Practical Training) for a month. Also at this time, I am not including my notarial no criminal records certificate. But I will gladly provide it should you require it.
