Thank you very much for your reply to my post regarding the J1 waiver case # application. But I am in shock totally after reading it! Why the "J1 Visa Waiver Review Application Instructions" I downloaed from DOS webpage only shows ONE address for both application fee and application form? Quoted:

Do you have any idea why it is different from what you described? I am so so desperate now. Even the easies (supposed) part of waiver is so hard for me!
