G-325A Form only has 2 pages chunshi2009-08-11 18:38:57

The G-325A from I download from USCIS site only has 2 pages (1 page form and 1 page instruction). I read PiJiang's blog, it says it should have 4 copies. Is this a new version? Has USCIS changed its requirement for this form?
