First H-1B: 12/7/2000-12/01/2003, and I started to work on 1/2/2001

Laid off in May 2003, and applied F-2 to keep status, F-2 receipt date: 5/9/2003,
Went to Mexico and got F-1 visa on 7/23/2003
Received F-2 approval with valid date from 7/26/2003

After graduation and OPT, got a new H-1B 6/4/2008-3/9/2011

Q: how many months I have as of 6/4/10 before reaching my H-1B's 6 year limit?
Is it 12 months based on the H-1Bs,
or 19 months (based on receipt date of F-2 on 5/9/03),
or 16 months (based on F-1 visa on 7/23/03)

I485 submitted in 2007, based on wife's NIW (PD 6/23/2006), but her working field is not exactly the same as that when she filed NIW.

What do I need to do before my current H-1B expires on 3/9/11? Renewal or extension?

Should I apply EAD based on I485? If I use EAD after 3/9/11, Can I switch back to H-1B in the worst case that the original 485 is denied?

Is it a good idea to ask my company to apply PERM as a backup plan?
