A review of your file does indicate that you submitted a medical eamination (Form I-693) completed by a designated civil surgeon. The medical examination was conducted on December 14, 2011, and the civild surgeon indicated that you had a Class B medical condition which has a material effect on Form I485. Because this medical condition has a material effect on the Application to Register Permancent Residence or Adjust Status (Form I-485) and Form I-693 is more than one year old, the validity of Form I-693 cannot be extended. Therefore, please make an appointment with a civil surgeon so that a new medical examination can be administered. Please note for medical examination completed on or after January 1, 2012, civil surgeons must use the current form (Edition dated October 11, 2011).
