I have just found the following words on the instruction of I-765:(you really need to read the instruction all) It is on the page 7, upper left hand side.

If your response to question 16 is (c)(9), file your application
at the same local INS office or Service Center where you
submitted your adjustment of status application.
****

hope this will help for those who confuse with EAD filing address change after Dec 1, 2004.
