I am going to apply for the J1 waiver. Could any one kindly tell me how I can apply the case number?
I found a J1 visa waiver review application form from the Dept. of State. It says that I need to fill this form and mail to St. Louis, MO along with other supporting documents and application fee.
My questions are:
1. Is this the form I need to fill to get the case number?
2. What are the other documents?
