I am going to Canada to get my H-4 visa. I saw some posts that said there will be a great chance for a visa application to be denied if one used to "out of status" when living in United States.

I had my H-1b Approval Notice(from 6/2001 to 10/2003). I was laid off in 6/2001 (don't laugh, I was laid off immediately after I got my H1), then I sumbitted H4 application within one week. Then it got approved, but the approves "received 11/2001, valid from 12/2001 to 8/2004". I didn't notice that untill I decided to go to get a visa from Canada recently. My questions are:
1) Was I out of status from 6/2001 to 12/2001?
2) Will the VO asked me to present my last pay stub or the Approval Notices are fine?
3) Why I submitted in June, 2001 but the approval notice says "received 11/2001?

I very much appreciate it if you could give me some suggestions or share your own experience. Miss home a lot and Worry a lot. I have been in US for 6 1/2 years ...
