About national award: I just have a thesis award. But I had it translated in a registered translating company here. I provided the original document to the translator. She verified, made copies, kept a record and notarized that. CIS does not like translation made by you or somebody you know.I did not send the original document. I did not provide the deion of the prize either as I could not find that.
About membership: I am an active member of AACR, and full member of SigmaXi. I did not remember SigmaXi membership requires nomination. However, it has to be reviewed and that takes about three months. AACR membership seems not working sometimes. but SigmaXi member does count, which I am pretty sure.
About reviewing service: You need the direct invitation from the editor. Email invitation is fine. But if you review on behalf of somebody, I am sure that does not count. But you can obtain such letters of support from editors from Chinese journal if you did reviewing work in China
Recommendation letters: I had five from professors in America and I avoided obtaining them from Chinese professors. But it seems that is not enough. I received RFEs asking for more comments of researchers who cited my work. I send about 20 emails requesting such letters of support, however, not many replied. But I finally got three.
Citations: I just printed what I got from the ISI web of science.CIS like those printed directly from from the web.
Contributions: The lawyers made my work and my role look important.
Degree: my Ph.D has an English translation which is issued by the University in China. If you do not have, I think a notarized translation will be fine. For my M.D, I just send copy of the ECFMG certificate.

Title at application: postdoc
I did two FPs, one is 2005 and one in 2005 (the previous post was wrong, second FP was done in 6/2006). My wife did with me at the same time for the first FP, I do not know why she did not receive the request for the second one.
I used AP to go back to China in January and renewed my H-1B recently. My wife also used her EAD to work. I think using EAD and AP are safe.
