I have some questions to ask for I-485 form?

1. What is the nonimmigrant visa number?
I used F1 visa to enter the USA, and this year got the H1B visa (I797A), should I use the Receipt # EAC-... for the visa number?

2. Were you inspected by a U.S. immigration officer?
Yes or No?

3. Consulate where visa was issued?
should I fill with "Immigration & naturalization servie/Vermont Center"?

4. List you present and past membership...
Should I write something like IEEE member? or just leave it blank? What is the real meaning of this membership?
