I am a H1B visa holder and submitted I-485 this July. My parents visited us this year and stay in US more than 190 days until now.I plan to apply ITIN for them.
1.In w-7 form,reason you are submitting form W-7. should I choose "d-Dependent of U.S. citizen/resident alien"? If I choose "d", I have to file tax return. If I want to get the ITIN before 2007 tax return, can I attach the 2006 tax return with the w-7 form now?(2006 I filed 1040).
we don't meet any exceptions,so could not choose "h".
2.In w-7 form instructions, a notarized or certified copy of a valid passport.Should I notarize all the pages in the passport, including passport, visa and entry and exit records? Or just notarize the passport page alone is OK?
3.In w-7 form ,other information-6d, if I choose passport, for the contents of "issued by", should I fill in "china" or other words?
