I first entered USA with J1 visa and went back to my home country and worked there for 3 years. last year I came to USA again with J1, now I would like to apply for J1 waiver. Should I enter all DS2019 forms information including formly IAP66 while applying for a case number? Will it be confused since my first J1 has been waivered automatically?
If I only need to input the current J-1 visa information, what should I fill in column 17 of the DS-3035 form? Which is about "the Date and place of first entry into the U.S. on your oringinal exchange visitor(J1) visa. Entry information should refer to the first time the J-1 visa was used to enter the U.S."
