Pjiang Teacher, help for RFE for I693, civil surgeon out of lice sunbeim2008-10-09 20:02:57

We received RFE for I693 requesting to finish the supplement form by the original civil surgon who performed the exam one year ago. Unfortunately this civil surgeon is out of license currently. What procedure or documents required to present to change to another civil surgeon to finish the forms? Is there any form? Thanks a lot.
