How to fill form (485) to change address desperate20002009-12-21 22:04:51

My 485 is pending. On-line I filled AR-11. During filling form for case 485 I’m not very clear what to fill:
- Should select “-- Applicant or Petitioner”?
- No place to fill attorney name?
- Beneficiary? Should be my name?
- For my child's case, Beneficiary Should be kid's name?
