我上月入籍同时申请了我和孩子的护照，我的护照10天后就收到了，孩子遇到了麻烦。因为我是离异前夫在中国，我后来跟美国人再婚，孩子三年前就有绿卡，申请护照时邮局也收走了绿卡原件。我并提交了，孩子的出生公证（中英文复印件），离婚公证（中英文复印件），法院判决孩子监护权公证（中英文复印件），前夫同意孩子来美跟我和先生的公证（中英文复印件），还另外填了一个表格
STATEMENT OF CONSENT OR SPECIAL CIRCUMSTANCES:ISSUANCE OF A PASSPORT TO A MINIO UNDER AGE 16
今天收到来信告知如下：
IN ADDITION:1,Court orders must be complete, and include the child's name and judg's signature
2,Notarized statement of consent must be signed on the same day by the non-applying parent/guardian in the presence of a certified notary public, be less than three months old, and accompanied by a photocopy pf the front and back side of the photo identification presented to the notary public.
问题，我还需要提交什么？到底是哪里出了问题？所有提交文件跟孩子申请绿卡的时侯是一样的啊。另外，
