supplemental form to I-693 oliva002005-05-10 05:12:27

I was asked to provide a supplemental form to I-693 which was not included by the doctor the first time. The form is about immunization record. However, the doctor did not check anything for the Hib, Hepatitis ect. this time. Is this right? Could anyone give me some comments about it? Should every item be checked?
