请教有经验的大侠：向美国国务院申请J1 waiver的case number时有一项statement of reason一般怎么写比较好，这一项重要吗？申请number时需要提供相关的材料吗？因为是网上申请，所以进行到这一项时拿不定主意就退出来了，等弄明白再写。
