I had a question for OPT approval:

did the USCIS send out the approval notice (I-797) for student optional pratical trainning with the OPT EAD card or only send you a OPT EAD card without a notice?

thanks; The reason I am asking is it had been a long time ago and I did not remember, USCIS ask for my approval date of OPT.
