Is the same as the job title when file I-140? And if so, the blank space is not enough to fill the words such as "Senior Software Engineer", how to handle this?

GIVEN YOUR NAME AS EXACTLY AS YOUR I-94 FORM:

My wife's name is three Chinese words, in I-94 form and H1-B visa there is a space between her first name's
first Chinese word pinyin and second Chinese word pinyin instead of written together without space.
For all her application and passport, her first name's first word and second word are typed together without space, what does she to fill for this field?
A. First name first word, space, first name second word, space and Last Name
B. First name first word first name second word, space and Last Name.
C Last Name, space, First name first word, space, first name second word,
D Last Name, space, First name first word, , first name second word,
I know this question seems funny, but it really bother her and me.

for my case, H1-B was issued in 2001 Beijing and landed at Seattle 2001
visit Canada several times since then and my original I-94 issued in 2001 was taken by INS officer when the first time I came back from Canada and was replaced with a new I-94 card date as 2004 at Blaine/WA (port of US INS office).

Based on Xiaobaitu's reply, I may can fill the last Entry as Blanine/WA instead of Seattle/WA
But the none immigration visa number, I still need to fill the H1-B visa that I got from Beijing, right?

Do I need to explain something about the above story when I fill these fields? if necessary, attach an amendment?
