Q1:What kind of form do I need to fill for H1-B, H4 in Guangzhou except DS-156, 157?

Q2:Have you ever been refused a U.S.Visa?

I got a stamp" Apllication Received" on my passport when I apllied my J2 visa first time. I got the J2 visa second time application. I am not sure which answer should I choose "yes" or "no".
