question about form I-765 Q 11 dates maomaohedoudoumama2007-07-25 20:26:38

question 11 on the form about the EAD date. I had OPT before, should I put the recived date on the receipt notice (OCT. 20, 2004) or the valid date of the OPT (1/10/2005).
