I came last year on a H1 visa, my I-129 petition was granted for the usaual 3 years, good till 9/2008. But due to the fact that my passport expires in 11/2007, when I entered the country, I was given a I-94 that is only good till 4/2007. I just renewed my passport, and I know I need to file I-129. Since I work for a very small company, I was told to go ahead and fill out the form myself.I am a little confused about filling the form. Does anyone have similar experience? Other than the $190 filing fee. Do I still have the pay the $750 fee and the $500 fraud prevention fee? What other documents do I need to send in? Labor Certificate? Proof of college degree? Since my original I-129 petition was granted till 9/2008, so what I am filing now is it considered to amend the stay or extend the stay? also, two Qs on the form, I don't know how to answer:
Is this the second or subsequent request for an extension of the stay that you have filed for the alien?
Is this an amended petition that does not contain any request for extension of stay?

I really appreciate your help. Thanks.
